When zAccess admin tries to delete or deactivate a user account or an admin user account whose First Name and Last Name fields are empty, clicking Delete or Deactivate for that account does not show any action or response.

Application Access > Users > Delete and Deactivate
Portal Management > Admin Users > Delete and Deactivate
Modify the First Name and/or Last Name of the account with something, then delete or deactivate the account.
